** The enrollment process must be completed by the Parent/Guardian of the student, or the student if emancipated. A separate form should be completed for each student being enrolled.
Before you fill out the initial enrollment form, please be sure to
- Check eligibility: Is the student a resident of South Carolina who is eligible for the 9th – 12th grade? The maximum age for enrollment is 21, and all students must graduate before their 22nd birthday.
- Review program information: If Provost Academy looks like the right school for you, take the next step by filling out the initial enrollment form below. If you still have questions about the school, academic program, etc. please feel free to contact an Enrollment Specialist at 877-265-3195.
How to Enroll in PASC
- Complete the initial enrollment form
- An Enrollment Specialist will contact you shortly to verify your entries, collect further detailed information, and discuss the next steps for submitting required forms and documents like proof of residency, birth certificate, immunization records, policy agreements, etc.
- Upon completion of all requirements for enrollment, you will receive an official confirmation of enrollment in PASC.
- In August, your Academic Advisor will meet with your family to discuss the student’s academic records and facilitate diagnostic testing which will be used to work together to create a customized academic plan for your student.

Please Note: Completing the initial enrollment form does not mean that your child is enrolled in PASC. There are additional steps to the registration process. Your child should stay enrolled in his/her school district until you have been officially notified that the enrollment process is complete and you have received an official start date.
